Overview of

Neodyumium - Yag Laser

in Thailand

Nd: YAG is an advanced laser technology available that is used for the treatment of a wide range of skin and eye disorders. Nd: YAG (neodymium-doped yttrium aluminum garnet) is a solid-state laser with neodymium as laser medium that emits an infrared wavelength (1064 nm), while in the Q-switched mode it emits 1064 and 532 nm wavelengths. 1064 nm is beneficial for deeper skin penetration while 523 nm is indicated for superficial treatments.

Laser (light amplification by stimulated emission of radiation) is a breakthrough medical technology. The laser concept is to produce a wavelength of high-energy light to penetrate deep into the skin to target specific cells through heat production which destroys these cells.

Multiple types of lasers are available, they differ in the types of elements that produce the light energy, wavelength, and depth of penetration which all determine the laser applications.

Goals of

Neodyumium - Yag Laser

Nd: YAG laser releases a high-energy light with a specific wavelength that creates heat and causes destruction of the targeted cells while being safe on the surrounding tissue.

The applications of Nd: YAG includes:

  • Treatment of vascular lesions such as spider veins in the face and legs, varicose veins, hemangioma, and telangiectasia (facial veins). The Nd: YAG laser with specific settings will target the hemoglobin in the treated area.
  • Tattoo removal. The Nd: YAG targets the ink molecules to be destructed and eliminated by the macrophages. The results are affected by the tattoo color and how deep the tattoo ink has penetrated.
  • Treatment of pigmented lesions such as brown age spots, solar lentigines, freckles, nevus of Ota, café-au-lait macules, and lip hyperpigmentation. The laser targets the melanin cells.
  • Hair removal on face, neck, and underarms. The Nd: YAG laser will target the hair follicle, it is more effective on light-colored skin with dark hair than the dark-colored or tanned skin with light hair.
  • Treatment of certain eye and nail disorders.

Pros and cons of

Neodyumium - Yag Laser

Pros

  • a non-invasive surgical-free alternative for the treatment of pigmented lesions, facial veins, hair removal, and tattoo removal.
  • it helps improve skin tone and stimulates collagen and elastin production.

Cons

  • it may require multiple sessions according to the severity of the treated problem. Tattoo removal requires multiple sessions while skin pigmentation due to aging may improve after 1-2 sessions.
  • ineffective for light-colored hair(blonde/grey).
  • Nd: YAG laser is used with caution in tanned, and dark-skinned patients, because it may destroy the melanin cells responsible for skin color leading to white patches.
  • it may lead to post-inflammatory hyperpigmentation in the treated area.

How it works:

Neodyumium - Yag Laser

  1. Before starting, your doctor will give you an eye protector (opaque covering or goggles) to protect your eyes throughout the whole process.
  2. A layer of numbing cream topical anesthetic will be applied to the area of interest to avoid any discomfort.
  3. According to the type of treatment, the Nd: YAG machine is adjusted in terms of wavelength and number of pulses.
  4. The laser machine handpiece is directed to the targeted area and the laser light is activated. The handpiece contains an integrated cooling system to avoid skin burn and discomfort.
  5. The treatment session duration varies from 5 minutes to hours according to the problem treated.

Preparation before

Neodyumium - Yag Laser

Pre - treatment

  • A careful examination and diagnosis for the targeted area are performed and the reason for any pigmentation disorders must be identified to avoid mistreatment of melanoma (skin cancer condition that appears as a pigmented lesion).
  • Skin color assessment is performed to make sure that the Nd: YAG laser is the most appropriate treatment.
  • One week before your session avoid using any retinol, benzoyl peroxide, or salicylic acid products because if these materials were combined with a laser skin irritation may occur.
  • Antiviral or antibiotic ointments may be prescribed if you are having an active infection or inflammation condition on site.

Recovery after

Neodyumium - Yag Laser

Post - treatment

  • Directly after the end of the session, an ice pack is recommended to be applied over the treated site to decrease any redness or discomfort.
  • Your dermatologist may choose to cover the treated site with a bandage in certain conditions.
  • It is recommended to avoid water use on the treatment site in the first three days.
  • Avoid wearing make-up for five days after your session.
  • Avoid using an abrasive skin cleanser in the first few days.
  • Antibiotic ointment is prescribed for 5-7 days after your session to avoid bacterial infection.
  • Mild swelling, redness, and itching may occur that will resolve in a few days. This can’t prevent you from continuing your daily activities as usual.
  • Daily sunscreen usage after treatment is recommended to avoid post-treatment pigmentation.
  • Healing time depends on the type of laser, session duration, and the energy level used.

Risks & side effects of

Neodyumium - Yag Laser

  • Post-treatment redness, swelling, itching, bruising, and black scabbing are common side effects after laser treatment. They resolve within a few days.

  • Improper use of laser may destroy melanin cells that are responsible for skin color which will result in either darker or paler skin patches. This is called post-inflammatory pigmentation.
  • Bacterial infection is a common side effect; it can be prevented by a topical antibiotic application after the session for 5-7 days.
  • This procedure is contraindicated if you are pregnant or planning to.
