Liposuction

in Thailand

Liposuction is a procedure that involves the removal of excess fat from under the skin. To do this the skin is either pierced with a cannula or treated with an ultrasonic probe, which breaks up fat cells, and then removed with suction.

Liposuction targets problem areas where there is stubborn fat that can’t be reduced through diet and exercise alone. By doing this the body can be sculpted and reduced into a desired shape. Liposuction can have long-lasting results and be achieved in just a few hours. The procedure can be done in more than one place in one session, for example the stomach, back and thighs can be treated in one day. Liposuction can also be used to reduce fatty tissue in men's breasts, also known as gynecomastia reduction, and lipomas, which are fat tumors.

Liposuction allows a person to trim down and sculpt chosen parts of the body. By removing these excess fat cells from specific areas the body can be shaped and contoured. Liposuction can achieve fat reduction in minutes with exact precision, compared to exercise and dieting, which can take a long time to yield results. Liposuction can also be done at the same time as other procedures, which means all of their issues can be tackled at once. Liposuction adds contour and definition to areas like legs, including calves, thighs and ankles, the stomach and buttocks, as well as a person’s arms, neck and chin. How it works:

Local anesthetic is administered to the target area. Anesthetic contains lidocaine, which causes numbing and epinephrine, which helps reduce blood loss. A cannula, which can vary from 2-5mm in diameter, is then interested into a small incision to begin fat suction. Once the fat removal is complete the incision is closed with stitches. The procedure generally takes 1 hour per surgical site.

The time of treatment will change depending on the size of the area being worked on and how much fat is being removed. Local anesthesia is used during liposuction and it can be combined with intravenous sedation. The surgeon will insert a narrow cannula into a small incision to help breakdown fat cells. The cells get broken down because the cannula is pushed and pulled through the fat and then suctioned up through the cannula. There are a couple of methods used in liposuction. The method described above is the ‘dry’ method. There is also the ‘tumescent’ technique which introduces fluids which aid in the breakdown of fat layers. A patient will be required to wear support or pressure garments around the treated areas to aid in recovery and yield the best results.

Preparation before

Pre - treatment

Prior to treatment a person will be asked to limit or stop habits that are considered to heighten risk factors. This includes smoking, drinking alcohol, taking hormone pills, herbal supplements and any other medicines your doctor tells you to stop. This will happen about 2 weeks prior to surgery. Those with prior medical conditions that may impact surgery will need clearance from their specialist. Having a proper diet and doing muscle-toning exercises prior to surgery will guarantee the best results post liposuction.

Before surgery patients will have an in-depth discussion about surgical techniques, potential complications, results, personal expectations and their preferences with their surgeon. Patients then undergo lab tests and other medical evaluations specifically chosen by their surgeon to determine if they’re suitable for surgery. The doctor will also change a patient's medication intake, if applicable, to  prepare them for the procedure. For instance painkillers like aspirin shouldn’t be taken about a week prior to surgery as they can cause increased blood flow and recovery time. Quitting smoking, even if only temporarily, reduces post-procedure issues. The consumption of anti-inflammatory drugs, painkillers, and herbal supplements should be stopped to reduce blood thinning complications during and after surgery. It is recommended that a patient be driven home rather than using public transport, and that they have someone helping them in the days following to reduce unnecessary strain post-procedure.

Recovery after

Post - treatment

  • Light to moderate exercise can be done the day following surgery.
  • A patient can do more vigorous activity after a steady increase 1 month post-surgery.
  • The treated area must be compressed in garments or bandages for 6 days to 2 weeks after surgery.
  • Ice packs and heating pads shouldn't be applied to the treated area.
  • Do not expose yourself to large amounts of water for a week. No baths, swimming, hot tubs or open water.
  • Make sure the incisions are clean.
  • A patient can shower 2 days after the procedure.
  • A patient must attend their follow-up appointments for check-ups and stitch removal.

Swelling and bruising will occur but will disappear in about 2 weeks post-surgery. The amount of discomfort felt will be impacted by the amount of areas treated and individual pain thresholds. Crucially, it is recommended that physical activity be limited for a month after the procedure. The wounds will mature over 6-12 months. 

It's important to take care of the body post-procedure. Following these recommendations will help with the healing process:

  • No strenuous physical activity following surgery. Exercise and activity can steadily increase about 4 days post-surgery.
  • To reduce swelling and bruising compression bandages or garments should be worn. Doing so also maintains the body's new contouring. It's possible to feel lightheaded when removing compression garments. If removing more than one garment leave a 10 minute gap between each to avoid this issue.
  • Drains may have been inserted to help with excess fluid drainage. Typically, the more drains there are the less bruising will occur.
  • Antibiotics can be taken according to instructions.
  • Incisions must be kept clean by either washing them or showering daily.
  • Do not use ice or heat packs on the treated areas during recovery.
  • Taking medication like aspirin and ibuprofen or any non-steroidal and anti-inflammatory drugs may increase bleeding and weaken the immune system.

Risks & side effects

Patient satisfaction post liposuction is high, however, it is still possible that issues may occur. All patients should be made aware of the limits and potential risks of liposuction. Each person should be made aware of the risks and benefits and consider them accordingly. Avoid feeling peer pressured into getting surgery. Decide on getting surgery based on deep consideration of the risks versus the benefits. 

Complications of Liposuction in Thailand

After all surgery there is a risk of infection. This is also true of liposuction. Surgeons will often prescribe antibiotics to liposuction patients to combat this. Incision sites must be kept as clean as possible during recovery to reduce the risk of infection.

It is possible to suffer from an embolism once fat starts breaking down during liposuction. This can occur if blood vessels are broken during surgery and fat enters the bloodstream. Fat can become stuck in the blood vessels, collect in the lungs or even the brain. Breathing difficulties can be a sign that fat cells have traveled to the lungs, this is known as a pulmonary emboli. If symptoms manifest post-surgery call an ambulance immediately, because fat emboli can cause disability and in rare cases, death.

Puncture wounds can also occur during surgery, this is known as 'visceral perforations'. This happens if the doctor can't see where the cannula is during surgery, increasing the risk of puncture or damage to internal organs. For example a person's intestines could get pierced during abdominal liposuction. Further surgical procedures will need to repair any damage. Visceral perforations can be fatal.

Serum can collect in areas where tissue has been removed. This condition is called 'seroma'. Serum is the fluid that makes up a person's blood.

Nerve compression and change in feeling. 'Paresthesias' causes a person's sense of feeling to change, often experienced around the surgical site. This can either present in increased sensitivity, which causes pain; or lack of feeling, which causes numbness. These issues can persist for weeks or months and in rare cases the changes in sensation could be permanent.

Swelling, also called 'edema', occurs after surgery but could persist for weeks or months past expected recovery time.

The skin surrounding a surgical site can die. Known as becoming 'necrotic' this happens when bacteria infect the surgical site. Necrotic skin can be identified through color changes and excessive skin loss.

If an ultrasound is used during liposuction it's possible to get burnt. This is because the ultrasound probe can get very hot.

Excessive loss of liquids during surgery can result in a fluid imbalance. Liquid is stored in fat, so its removal contributes to this loss of liquids. Also, some surgeons will inject liquids to help breakdown fat which can cause an imbalance. Patients are monitored for signs of fluid imbalance during their hospital stay. However, the issue could start once the patient is home, causing fluid retention around the heart, lungs or kidney issues.

Lidocaine is injected to case numbing and too much could be used. Signs of too much lidocaine being used include, dizziness and lightheadedness, sleepiness, slurred speech, numbness of the lips and tongue and muscle twitching and spasms.

Complications arising from liposuction procedures are often infrequent and typically trivial. Of course all surgery carries risk, some risks related to liposuction include:

  • Hematomas, which is where bleeding occurs under the skin.
  • Scarring, though minimal, can take up to a year to heal and also be red and bumpy.
  • Infection may happen which will need antibiotic treatment. Although the risk of infection related to liposuction is minimal.
  • Uneven skin contour and skin depressions.
  • Pulmonary edema can form when ‘wet’ liposuction is performed. This is due to the copious amounts of fluid used in the procedure. This fluid may also cause an allergic reaction. 
  • A negative reaction to anesthesia.
  • The body’s symmetry may be off balance.
  • On rare occasions numbness and swelling can persist months after surgery.

Who are the best candidates for Liposuction?

Those best suited to liposuction are people who are ‘normal’ in weight, but have an issue shifting fat from certain areas. Having healthy skin, which is firm and elastic, will yield better results. Ultimately it will be the doctor that decides the suitability of a patient, this will be after an examination and consultation have taken place. Individuals that have underlying health conditions or bleeding disorders will likely be deemed unsuitable.

 

Will I be conscious during the liposuction procedure?

General anesthesia will be used. Patient’s will not be conscious.

 

Will liposuction on my tummy make it perfectly flat?

Liposuction removes fat from under the skin, not from the abdominal cavity. You will experience a slimming effect, but your belly won’t be completely flat. 

 

Will results be permanent after the liposuction surgery?

Good diet and exercise is key to maintaining your body after liposuction. Anything that causes fluctuations in weight risk undoing the results. Liposuction helps shape the body, it is not a replacement for weight loss.

 

Will there be scars after the procedure?
